Answer activity
You can use a comment to answer an activity.

Click PPM > Modules > Improvements.
Click an activity in the Continuous business improvement list.
Display the Base tab bar.
Click the Answer button.
Write your answer.
In the Link box you can specify a URL for a document containing more details.
Click OK.
The PPM users affected can see in the Reactions column that there is one or more reactions relating to their activity, and can view these in the comment's detail view.
